Required Documents

Based on the type of Special Share or Business Share Account you are applying for, the documents listed below are needed before the account may be opened. Documents may be returned via fax, scan & emailed or mailed back to  SFFCU.

Note: All accounts will need the account card signed (wet signature) and returned prior to the account being activated.

Business Share Accounts

Required Documents – Business

Submit with application:
  • Articles of Incorporation
  • ID for all Officers
Submit after SFFCU sends to you:
  • Corporation/Business Entity/Organization Membership Statement
  • Beneficial Ownership Certification
  • Account Card
Submit with application:
  • Partnership Agreement
  • ID for all Partners
Submit after SFFCU sends to you:
  • Corporation/Business Entity/Organization Membership Statement
  • Beneficial Ownership Certification
  • Account Card
Submit with application:
  • Articles of Organization
  • ID for all Managers
Submit after SFFCU sends to you:
  • Corporation/Business Entity/Organization Membership Statement
  • Beneficial Ownership Certification
  • Account Card
Submit with application:
  • ID for Primary Owner
Submit after SFFCU sends to you:
  • Corporation/Business Entity/Organization Membership Statement
  • Account Card

Special Share Accounts

Required Documents – Special

Submit with application:
  • Certificate of Trust or an Affidavit of Trust
  • IDS for Trustees
Submit after SFFCU sends to you:
  • Trust Account Worksheet Application
  • Information to Facilitate Trust Account
  • Account Card
Submit with application:
  • Organization or Club Bylaws
  • ID for Applicant
Submit after SFFCU sends to you:
  • Corporation/Business Entity/Organization Membership Statement
  • Account Card
Submit with application:
  • Letter from the Social Security Administration. The letter includes the following:
    • Representative Payee Name
    • Recipient Beneficiary's:
      • Name
      • Social Security Number (SSN)
      • Date of Birth
      • Gender
      • Address
      • Other pertinent informtaion
    • IDS of Representative
Submit after SFFCU sends to you:
  • Account Card
Submit with application:
  • Letters of Office for Executor or Administrator; Signed by Judge to Open Probate Estate
  • ID for Authorized Person Handling the Estate
